The NextGen Bridge Program helps patients transition safely from hospital to home by providing free, short-term skilled nursing visits.

Our nurses step in when there’s a change in condition — such as a recent hospital stay, new medications that require teaching, or when closer monitoring and side-effect checks are needed.

The program focuses on short-term recovery and stabilization, ensuring patients and families receive the right support during vulnerable times — without financial burden

Bridge Program Services

The NextGen Bridge Program provides short-term skilled nursing visits at home, tailored to meet immediate clinical needs after hospitalization, new medications, or changes in health. Our nurses deliver care that follows Medicare certification standards, helping patients recover safely, stabilize quickly, and avoid unnecessary hospital readmissions — without financial burden.

Comprehensive Nursing Assessments & Vital Sign Monitoring

Regular RN visits provide recovery-focused assessments, ongoing monitoring, and timely interventions to keep patients safe, supported, and on track toward stable health outcomes.

Pain Management & Post-Hospital Recovery

Nurses support safe pain control through education, side-effect monitoring, and comfort strategies, while also assisting patients after hospitalization to promote healing, stability, and reduced risk of readmission.

Injections (Insulin, Intramuscular, etc)

Nurses administer prescribed injections such as insulin or other IM/SC medications, while also providing education, monitoring for side effects, and supporting safe adherence to ongoing treatment plans.

Who Qualifies for the Bridge Program?

Patients must have a qualifying medical need for skilled nursing, such as medication teaching, injections, vital sign monitoring, chronic condition management, or recovery support after hospitalization.

Who qualifies?
Post-hospital change in condition, new/changed meds needing teaching, vitals/symptom monitoring (CHF, COPD, DM, HTN), injections, short-term stabilization.

It follows Medicare certification standards and reflects our commitment to becoming a Medicare-approved provider. While nursing visits are free, insurance may be required for medications or medical supplies. Services are offered for a limited time, subject to eligibility, and available on a first-come, first-served basis.
